About this role
The Care Coordinator will work closely with GPs, practice teams and other health, social care and voluntary-sector professionals to identify and support patients who would benefit from coordinated, personalised care. This may include people living with long-term or multiple health conditions, frailty, disabilities, complex needs or health inequalities. The post holder will manage a defined caseload and act as a central point of contact for patients, families and carers. They will help individuals navigate health and care services, understand the support available to them and become more actively involved in managing their own health and wellbeing. The Care Coordinator will bring together information about each patients health, care and wider support needs, helping to develop and review personalised care and support plans based on what matters to the individual. They will facilitate communication between services, follow up agreed actions and help ensure that patients receive joined-up and timely support. The post holder will work as an integral member of the multidisciplinary team and will collaborate particularly closely with social prescribing link workers, health and wellbeing coaches, community teams, secondary care and voluntary-sector organisations. This is a non-clinical role. The post holder must work within the boundaries of their competence and refer clinical, safeguarding or other significant concerns promptly to an appropriate clinician or senior member of staff.
