About this role
Provide rehabilitation support to palliative care patients, helping them achieve their goals and maintain independence. Deliver delegated rehabilitation interventions, support the safe use of equipment, assist with group programmes and home visits, monitor and report patient progress, maintain accurate records, and work collaboratively with the multidisciplinary team to deliver high-quality patient care. Participate in training, supervision, and ongoing professional development.
