About this role

We are looking for an enthusiastic and highly motivated MDT Team Lead to join our Specialised Cardiology directorate. This will include co-ordinating weekly multi-disciplinary team meetings where diagnosis is discussed and treatment planning decisions are taken, monitoring and recording outcomes at each stage and following up on all outcomes of MDT.

The MDT co-ordinator’s post is to provide dedicated administrative support to Specialised Cardiology Performance and multi-disciplinary teams (“MDT”), working together to provide high quality care for patients. The post holder will be expected to be proactive in seeking feedback from clinical colleagues and initiate changes to team working practices to adapt to the CAGs priorities and the clinical team’s requirements.

Key working relationships are with patients & carers, GPs and members of the internal administrative team and clinical teams including managers, administrators, nursing and medical staff, allied health professionals and support workers. They will frequently liaise with a range of staff internally across the sites/Trust and with partner and external NHS Trusts.

This is a key role with specific responsibilities to support the Lead Clinicians and multidisciplinary team in identifying patients for discussion at the weekly specialist MDT meetings. MDT coordinators may cover one or more MDTs, as deemed appropriate.

  • Co-ordinating weekly multi-disciplinary team meetings where diagnosis is discussed and treatment planning decisions are taken
  • Provide a high standard of administrative report to the clinical teams
  • Escalating to Services when next steps in pathways may delay diagnosis or treatment
  • Providing analysis of when patients are not treated on time, to support service improvement.
  • Monitoring and recording the outcomes at each stage in the patient’s journey with particular reference to pathway tracking
  • Following-up on all actions / outcomes of the MDT at each stage of the patient’s pathway.
