Band 6 FIND Team Care Coordinator - Bristol

The post holder will provide specialist assessment, care coordination, intervention and risk management for adults with Learning Disabilities and/or Autism who present with complex forensic, mental health and behavioural needs. Working as part of a multidisciplinary team, the role will involve supporting individuals transitioning from secure and inpatient services into community settings, promoting recovery, independence and positive outcomes. The post holder will work collaboratively with a range of health, social care, inpatient and third-sector partners, contribute to safeguarding and public protection processes, and provide specialist forensic advice and consultation where appropriate.
